Good day everyone!
Please take note of the things to bring on the examination day!

  • Notice of Admission
  • Application Stub
  • PRC Official Receipt
  • Two or more pencils (No. 2)
  • Ballpens with BLACK INK ONLY
  • One (1) piece Metered-Stamp Window Envelope
  • One (1) piece Long Brown Envelope
  • One (1) piece Long Transparent (non -colored) Plastic Envelope (to keep above items)



Included in the pack is a prayer to St. Joseph of Cupertino

For the pencil, I used in the actual Board exam the Faber-Castel Exam Pack 
which includes:
  • 2 pcs. Catell 9000 2B (Pencil)
  • 1 pc. ersaer specially formulated for exam 
  • 1 pc. sharpener


Tips in achieving successful exams included also

In addition to the list above on what to bring, it is important to bring candies, biscuits and water to keep you going since the exam is really very draining. You may also want to bring a packed lunch if you want to save money and would not want to spend on Fast Food. If you will be assigned in a private school, better bring a jacket for you might be seated beside an air conditioner. But if ever you will be assigned in a public school, expect to feel the summer specially for the JUNE takers, LOL!....bring your handy dandy hanky, tissue or towel and also bring a fan/ "abanico". I was assigned at a public school before but it was December so fortunately it isn't that hot anymore.

Initial Nurse registration...Late



Just want to share my experience last March 6, 2013, the day that I registered as a professional nurse. The schedule of my registration was suppose to be on February 18 but due to many reasons, I registered late. Anyways, I saw and copied from the internet some requirements for the registration like:
    Oath form (downloaded from the net then print and fill up)
    1 documentary stamp
    1x1 picture with plain white background and complete name tag,
    cedula (should be recent meaning, this year),
    


      First, I went to True Colors Photo shop in Paco for a 1x1 photo with name tag (12 pcs for 50 Pesos) but unfortunately there were lots of customers so I waited for an hour. 
  
      
      I arrived at PRC at 12:07, I still don't have cedula at hand but they have it there inside their vicinity. I don't want to buy those outside cause what I know is that it's fake.
Unfortunately, it was break time for the Cedula booth, so I waited until 1 o'clock. While waiting, I ask at window Q for an Action Sheet to be able to request for certificate of Rating and certificate of Passing. I filled up the form then went to the cashier beside the food court...75 pesos per copy. How many action sheet is needed for the certificates? 1 action sheet only for both certificates. After paying, I went to window K, gave them the form and they will give you the claim stub. The release of my certificates was March 11. 

At exactly 1o clock I queued for the cedula, price was 5.30 Pesos but he got my 6 pesos...(Damn! He didn't give me my change. LOL!)
      
          I showed my oath form (not notarized) at Window 19. They attached the registration record that I filled up before when I applied for the board exam.


     I brought the forms to the cashier and paid a total of 1050 pesos. The girl at window 19... she told me to tell the cashier to not forget to "tag" my document . So don't leave the cashier until you have been "tagged" because for sure the girl from window 19 will make you go back to the cashier. 
    

     I returned to window 19. She made me sign... 3 signatures plus  a thumb mark. Then she gave me my claim stub. I was giving her my 1x1 and passport size photos w/ name tag, the documentary stamps and my cedula, but she told me that she don't need to get it. WT Heck!!! I wasted my time and money to get those. LOL


     I'm NOT suggesting you NOT to bring these requirements ok! It's up to you. Just bring these to be sure.

NURSING BOARD EXAM: What to do after filing on-line


Good afternoon y'all!

What will you do after filing your application for examination online?
Here's what I did. (based on my experience)

I went to PRC and brought all the requirements.
 here's the list.


Documentary Requirements

  1. Original and photocopies of Transcript of Records with Special Order and Date of Graduation, with scanned pictures and with remarks “FOR BOARD EXAMINATION PURPOSES ONLY”. 
  2. Original and photocopy of NSO-issued Birth Certificate 
  3. For married females, original and photocopies of NSO-issued Marriage Contract 
  4. Two (2) passport-size colored pictures with white background and complete name tag
  5. Current Community Tax Certificate (Cedula)
  6. Summary of RLE (no need to photocopy, they'll get the orig)
  7. If you're a graduate of a private school, you should request for a Certificate (Affidavit of Undertaking) from your school that you have already completed your cases  (Ipa-notaryo mo, they will also get the orig no need to photocopy)
  8. In addition bring your Cedula 
  9. Bring also an ID (hindi kailangan but just in case)

How to go to PRC...
      From Makati, ride a jeepney going to Paco/Taft/Herran. Get off at Taft. There are buses going to Fairview and tell the driver you are getting off at PRC. Fare is 10pesos, I rode an ordinary bus. Just tell the driver or the "konduktor" that you are getting off at PRC.
When you are there, you won't be able to see the PRC building because you still need to walk towards Paredes St. to get there. Don't worry, it will be just a short walk.

When you get inside the building, go to the customer assistance window to buy 3 pcs. of documentary stamp which is 21 pesos EACH.

You may also want to buy the metered stamped envelope at the PRC coop, priced at 21 pesos. (Your facilitator will ask this from you on your examination day!!!)

Then go inside the building beside the Cashier. There are windows on the left side and on the right side inside the building. Go to the right side, those are your windows. But there is a queue,  you may want to ask the guard where you should go. The guard was also the one who collected our photos (1 pc passport size) for scanning. Write your full name behind the photo.

When I reached the window 1, thank God!, they assessed my documents.
The girl saw that I had a discrepancy with my middle name, it had a typographical error. (It's because in all my documents, I have been using my correct middle name).  Shoot...I was afraid that I might not be able to take the exam because of this but good thing, all I needed to do was to go up the legal office to request for an Affidavit. If I remembered it right, I paid 120 pesos. But before I went up, she already gave me the permanent exam card that you need to fill up and the printed application form that you filled up online.

After getting the affidavit, I went back to window 14, she told me go to the cashier to pay for the fees. 900 pesos exam fee.

After paying, I brought all my papers to window 14 she signed it then she gave me the papers and I passed it to the window at the end near the stairs. I waited until FINALLY, my NOA (notice of admission) has been released.

NURSING BOARD EXAM


How do you apply for a Nursing Licensure examination?

First, apply on line...

It is really nice that now a days you may file your application on line. Just go to the PRC official website, click HERE, but first you need to get your cedula because it is required in the form. You may buy your cedula at Manila city hall (for those who live in Manila areas). It costs 5.40 - 6 Pesos in Manila... actually if you're from Manila you may also buy it from Makati city hall. (at a much higher price).

If you have not filed online, you may go directly to the PRC and bring all the requirements. But they will ask you to go up the 5th floor to file online. 

For married women filling up the form online... for the LAST NAME, write the surname of your husband, the one you are using now. On the NAME, common sense, of course your name.  On the MIDDLE NAME, your surname when you were not yet married/ or the surname of your father. Then for the MAIDEN NAME, write in there your mother's surname when she was single.

EX. Marissa Cruz Martinez-David (David is the married name) So you are Marissa M. David, now.

Last name:       David
       Name:             Marissa
       Middle name:   Martinez
      Maiden name:  Cruz      ( this is the confusing part, because when you say Maiden name... that would be your surname when you were still single right???) But well... it appeared correct on the Notice of Admission when I went to PRC to pass the requirements). They just did not explain to me that that is just the way they fill it up.

After submitting the form, you will receive an approval, and then you may go to PRC to submit the necessary documents.
